EU-wide disability railcard?

No matter what I do on Google, the only information I can uncover about an identity-style card for long-distance trains for those with a physical disability/mobility impairment (such as myself) is information about the Disabled Persons Railcard in the UK, which I already am in possession of.

Does an identity card for the European continent (EU or EU-plus-whoever) for disability even exist? And if it does, where is the link to the application form to apply for one? I want to be able to qualify unequivocally for reduced-fares across all Europe’s train systems, without getting hassled, and an ID card would help tremendously with that. Thanks for any and all help!

In Germany, if you show your card that verifies that you are disabled (UK card) when you buy a ticket, you will get the German disabled discounts that are available to the Germans.
I am doubtful that an EU wide card exists but am convinced that most EU states will offer you discounts or additional services if you show your UK card.
